Our Services

We can help with your NDIS Plan.

You may have a plan in place or require a Service Provider, or you may need assistance linking in with community activities or services, either way we are here to help.

Service 1

Community access : Helping people with transportation, group activities, access to community events and services, assisting with networking and creating new friendships.

Service 2

In Home Support : Either in the comfort of your home or one of our Homes, we can provide assistance with daily living and personal care, cooking, cleaning, health and well being, recreation, entertainment, life skills and making the boring everyday routine a lot more fun.

Service 3

Accommodation for people with disability, respite for families and participants and community nursing. Including , SDA, SIL & RESPITE

Community nursing ,
Qualified nurses available to assist you in the comfort of your own home

We have access to a rural respite centre located in close proximity to Cradle Mountain, which enables us to provide respite and getaways for our clients to relax , get back to nature, enjoy bush walks, camp fires, wild life and recharge their soul.

We have more than 20 years experience in the disability sector and we are determined to leverage that knowledge and give a little back by providing a friendly professional service that puts people first.

We aim to support people in their own homes and in the community. and offer a service that enables us to deliver a holistic approach to ensure our family at A.P Horizon are well looked after.

We are registered to provide Disability Support Services in home and in the community. We have our own registered nurse to provide community nursing as required, transportation, specialist plan and behavioural management.

We currently have 20 + years experience in the disability sector with a diverse range of roles from support workers, nursing, plan management, and Managing Directors.

We have always enjoyed helping people live their best life, giving people the opportunity to explore, dream and achieve anything.
